In order to apply for a new passport or replace one that was lost or stolen, you should present the following:

Original Ecuadorian identification card ("Cédula"). This document should not be more than 12 years old; if you do not have the original "Cédula" you should request a certified copy from the Civil Registry in Ecuador called the "Tarjeta Indice."
In this case, the Consulate needs to prepare a sworn statement regarding the loss of your passport. This document costs $20.00.
Total cost for the passport is $110.00 (in cash or money order).

Important Notes:

Ecuadorian birth certificate cannot be presented as a replacement for an Ecuadorian identification card ("Cédula") as neither the applicants picture nor signature appear on the document.
In order to obtain a new passport, the applicant must personally request it at the Consulate.
New passports can be picked up by the applicant in approximately 3-10 business days. Applicants may take a self-addressed, pre-paid envelope to the Consulate for the new passports to be mailed out when ready.
